Camberwell Medical Group welcomes new patients!

New patients require a 30 minutes appointment, as the doctor needs the extra time to get to know you, and your medical and family history.

 

We ask all new patients to bring along a list of all their current medications they are taking with them.

All new patients are required to fill out a new patient registration form on first visit. Please try to arrive 5-10 minutes early for your first appointment to complete the new patient registration form.

Don’t forget to bring your photo ID and Medicare card to your first visit.
